What You'll Be Doing
Client Assessments & Care Planning
- Meet with prospective clients and families to assess care needs.
- Develop personalized care plans.
- Coordinate service starts and work with scheduling to ensure proper staffing.
- Provide hands-on caregiving support when operational needs require.
Hospital & Facility Care Coordination
- Coordinate transitions from hospitals, rehabilitation centers, and SNFs.
- Collaborate with social workers, discharge planners, case managers, and family members.
- Help reduce hospital readmissions through proactive follow-up.
Quality Assurance & Client Satisfaction
- Conduct satisfaction calls and in-home quality visits.
- Perform reassessments and update care plans.
- Ensure Assignment of Benefits forms are completed and maintained.
Caregiver Coaching & Support
- Conduct field visits and caregiver observations.
- Assist with onboarding and training.
- Provide coaching and additional education as needed.
- Support caregiver engagement and retention.
Client & Family Relationship Management
- Serve as a trusted resource for clients and families.
- Respond to concerns with empathy and professionalism.
- Encourage positive reviews and referrals.
Required Qualifications
- Active CNA, CHHA, or HCA credential
- High School Diploma or GED
- Valid driver license, reliable vehicle, and proof of insurance
- Minimum one year of caregiving experience
- Comfortable traveling throughout the service area
Preferred Qualifications
- Minimum one year of caregiving experience in home care, home health, assisted living, memory care, skilled nursing, or related healthcare setting.
- Experience performing client assessments and creating care plans
- Experience working with hospitals, discharge planners, case managers, or social workers
- Experience training, mentoring, or leading caregivers
